Attestata dal 1227, sorge in luogo della chiesa di San Salvatore in Porta Aurea, antica porta delle mura altomedievali.
Ad aula unica, fu ristrutturata nel XVI secolo. La facciata del 1757 è di Ignazio Pellegrini. All'interno, a navata unica dotata di una cappella su ogni lato, vanno segnalate il soffitto ligneo dorato del 1642, attribuito a Del Norcia, e le tele di Jacopo Vignali, Cecco Bravo, Lorenzo Lippi e Francesco Curradi. Il nome della chiesa deriva dall'immagine affrescata della Madonna col Bambino attribuita a Taddeo di Bartolo , trovata nel 1640 nelle case dei Galletti, che oggi si conserva sull'altare maggiore.
