Wustrau-Altfriesack ligger ved bredden af Bützsee og Ruppiner See i Ruppiner Land. Bydelen omfatter delene Altfriesack, Wustrau og Zietenhorst. Den bliver krydset af statsvej 164. Jernbanelinjen Kremmen–Meyenburg løber nordøst for kommuneområdet.
Altfriesack var oprindeligt en fiskerby, der første gang blev omtalt i dokumenter i 1421. Den middelalderlige bykerne ligger på en ø i forbindelsen mellem Ruppiner See og den sydligt liggende Bützsee. I den sydlige del af øen fandtes der i middelalderen med Burgwall Altfriesack sandsynligvis et kultisk centrum for den slaviske stamme Zamzizi. I Neues Museum Berlin findes Altfriesacker Götze, figuren af en slavisk pælgud, der blev fundet i Altfriesack.
