Det var et af de områder, der var underlagt det tidsmæssige herredømme af biskopperne i Tortona, hvorfra navnet stammer. Som hele området var det genstand for de ekspansionistiske mål fra Hertugdømmet Milano, og det var teater for jurisdiktionale konflikter mellem biskopperne og den spanske regering først og den savoyardiske regering senere. Efter den napoleonske periode blev det endeligt et territorium i Kongeriget Sardinien.
