Mühlheim an der Donau liegt im Herzen des Naturparks Obere Donau rund zehn Kilometer nordöstlich der Kreisstadt Tuttlingen. Im westlich von Mühlheim liegenden Ortsteil Stetten mündet der Kesselbach in die Donau. Im Bereich der Altstadt mündet die Lippach, und nahe am östlichen Gemarkungsrand der Wulfbach.
