Die Bahnstrecke Mailand–Genua ist eine italienische Bahnstrecke der Rete Ferroviaria Italiana . Sie ist 157 Kilometer lang und eine Hauptstrecke. Der Personenverkehr wird hier von Trenitalia betrieben.
Im Gegensatz zur Bahnstrecke Turin–Genua entstand die Strecke Mailand–Genua aus einem Zusammenschluss bestehender Streckenabschnitte. Einer dieser Abschnitte war die Strecke Rogoredo–Pavia, die von der Strecke Mailand–Piacenza abzweigte und am 10. Mai 1862 in Betrieb genommen wurde. Zuvor, am 25. Januar 1858, waren die Strecken Alessandria–Tortona–Voghera–Casteggio und Tortona–Novi Ligure eröffnet worden. Diese stellten eine Verbindung zur bereits existierenden Bahnstrecke Turin–Genua her. Am 14. November 1867 wurde die Verbindung zwischen Genua und Mailand mit dem Abschnitt Pavia–Voghera vervollständigt.
