Il territorio comunale, dove cui si uniscono la Valle del Tirino e la Val Pescara, è compreso nell'area del parco nazionale del Gran Sasso e Monti della Laga.
Il paese è attraversato dal fiume Tirino che nasce dal sistema acquifero di Campo Imperatore e dopo un percorso carsico di 25 km viene alla luce attraverso tre sorgenti, tutte nel territorio di Capestrano: sorgente di Capodacqua, sorgente di Presciano e sorgente del Lago. Lungo il suo percorso viene alimentato anche da altre piccole risorgive. Il suo nome proviene infatti dal greco "tritano" che significa triplice sorgente.
Attualmente la portata è di circa 6000 litri/secondo e la temperatura delle acque di 11 °C.
