De kerk werd in de tiende eeuw door bisschop Heraclius gesticht op een eiland dat omsloten werd door twee armen van de Maas. Zij was tot aan de Franse Revolutie een van de zeven kapittelkerken in de stad. Bij het Concordaat van 1801 werd de Sint-Pauluskerk verheven tot de nieuwe kathedraal van het bisdom, aangezien de oude Sint-Lambertuskathedraal vanaf 1794 was afgebroken tijdens de Luikse Opstand.
