Barntrup ligger i den østlige del af Lippe kommune og regionen Ostwestfalen-Lippe i Teutoburger Wald/Eggegebirge naturpark, og dets byområde grænser mod øst op til den nederlandske kommune Hameln-Pyrmont. Kernbyen ligger cirka 5,5 km nord-nordøst for Blombergs kerneby og omkring 17 km øst-nordøst for Detmold. Sydøst for Barntrups kerneby udspringer Bega, som løber vestover fra byområdet, og øst for Grießebach, som løber nordøst, samt vest for bydelen Alverdissen Exter, som løber mod nord.
