Stemshorn ligger i Naturparken Dümmer, ikke langt fra Dümmer og Stemweder Bjerg mellem Osnabrück og Bremen. Kommunen tilhører Samtgemeinde Altes Amt Lemförde, som har sit administrationskontor i byen Lemförde.
I området omkring Stemshorn har der allerede været bosættelse i yngre stenalder og bronzealder. Dette fremgår af resterne af de fem præhistoriske brønde i Stemshorn, som i 2017 blev opdaget cirka 900 meter nordvest for byens kerne under rørledningsbyggeri på et markareal.
