Stemshorn est situé dans le parc naturel de Dümmer, non loin du Dümmer et de la montagne de Stemweder, entre Osnabrück et Brême. La commune fait partie de la collectivité de communes d'Altes Amt Lemförde, qui a son siège administratif dans le hameau de Lemförde.
Dans la région de Stemshorn, il y a eu une occupation déjà pendant la période néolithique et l'âge du bronze. Cela est attesté par les restes des cinq puits préhistoriques de Stemshorn, qui ont été découverts en 2017 à environ 900 mètres au nord-ouest du centre du village lors de travaux de pipeline sur un terrain agricole.
