Grayan-et-l'Hôpital er en fransk kommune med 1.486 indbyggere i departementet Gironde i regionen Aquitanien. Grayan-et-l'Hôpital ligger i nordenden af Médoc-halvøen, i området Landes de Gascogne, mellem Atlanterhavet og Gironde-flodens udløb af floderne Garonne og Dordogne. Kommunen består af hovedbyen Grayan-et-l'Hôpital og en række små bosættelser. Afstanden til det sydøstligt beliggende Bordeaux er knap 90 kilometer. De overvågede strande ved Atlanterhavet, ved Côte d'Argent, ligger cirka fem kilometer fra byens centrum. Kommunens område tilhører den regionale naturpark Médoc.
