Opperzau liegt nordöstlich in der Gemeinde Windeck, direkt an der Sieg und nahe einem Waldgebiet. Gemeinsam mit dem rheinland-pfälzischen Oppertsau bildet Opperzau ein zusammenhängendes Siedlungsgebiet.
Ein 200 Meter langes Stück der Landesstraßen 112 / 267, das Opperzau und Oppertsau trennt, stellt nicht nur die Gemeindegrenze dar, sondern ist zugleich auch die Landesgrenze zwischen Nordrhein-Westfalen und Rheinland-Pfalz . Zudem führt die Straße auf jeder Seite einen eigenen Namen: Siegstraße für die Oppertsauer und Fürthener Straße für die Opperzauer Seite.
