Die Operation Flavius wurde von Soldaten des Special Air Service , einer britischen Spezialeinheit, in Gibraltar am 6. März 1988 durchgeführt. Daniel McCann, Seán Savage und Mairéad Farrell von der Active Service Unit , eine Abteilung der IRA, bereiteten den Anschlag mit einer Autobombe anlässlich eines Wachwechsels vor der Residenz des Gouverneurs von Gibraltar vor. Während der Vorbereitung des Anschlags wurden die drei Personen von einer SAS-Einheit erschossen. Dies soll geschehen sein, weil der SAS falsch informiert war und annehmen musste, dass die Bombe bereits platziert und zur Detonation über eine Fernzündung vorbereitet war.
