Der Bauauftrag für das Boot wurde am 15. Oktober 1940 an die Germaniawerft in Kiel vergeben. Die Kiellegung erfolgte am 16. Juli 1941, der Stapellauf am 14. März 1942, die Indienststellung unter Oberleutnant zur See Hans-Hartwig Trojer am 9. Mai 1942.
Am 1. September 1942 lief U 221 um 7.00 Uhr von Kiel aus und lief am 22. Oktober in St. Nazaire ein. Während dieser Feindfahrt lief U 221 am 2. September in Kristiansand ein. Dort wurde nochmals Proviant und Wasser ergänzt. Am 3. September lief das Boot aus. Es operierte im Nordatlantik und östlich von Neufundland. Am 3. und 4. Oktober wurde U 221 noch mit 60 m³ Brennstoff und Proviant für 14 Tage versorgt. Am 22. Oktober um 18.30 Uhr lief das Boot in St. Nazaire ein. Während dieser Feindfahrt wurden insgesamt fünf Schiffe mit 29.681 BRT versenkt. Die Feindfahrt dauerte 50 Tage. Dabei wurden 7.588,6 Seemeilen über und 422,7 sm unter Wasser zurückgelegt. U 221 wurde für die Zeit der Operation den U-Boot-Gruppen Pfeil, Blitz, Tiger und Wotan zugewiesen.
