El submarino fue puesto en servicio en 1963 en la Royal Navy y sirvió allí en el primer escuadrón de submarinos.
La Otus estuvo en servicio, entre otras ocasiones, durante la Guerra de las Malvinas y en el Golfo Pérsico. Después de 28 años de servicio, la Otus fue retirada de servicio en 1991. Más tarde, el submarino fue comprado por un empresario alemán. Tras su traslado a Alemania, fue reparado en la Volkswerft Stralsund y recibió una nueva pintura en su color original. Desde entonces, la Otus se puede visitar como museo de submarinos "Erlebniswelt U-Boot" en el puerto de la ciudad de Sassnitz.
