La diócesis de San Marino-Montefeltro es una diócesis católica romana ubicada en Italia y San Marino, con sede en la ciudad de Pennabilli. La diócesis pertenece a la provincia eclesiástica de Rávena-Cervia, y es, junto con las diócesis de Cesena-Sarsina, Forlì-Bertinoro y Rímini, sufragánea del archidiócesis de Rávena-Cervia.
La diócesis de Montefeltro fue fundada en el siglo IX. Era sufragánea de Rávena. En 1154, la diócesis fue colocada como immediatum bajo la autoridad directa de la Santa Sede. El 4 de junio de 1563, la diócesis de Montefeltro se convirtió en sufragánea del archidiócesis de Urbino. El Papa Gregorio XIII trasladó el 25 de mayo de 1572, con la constitución apostólica Aequum reputamus, la sede de la diócesis de San Leo a Pennabilli. El 22 de febrero de 1977, el nombre de la diócesis fue cambiado a San Marino-Montefeltro y nuevamente sufragánea de Rávena.
