La Basílica de Santa María delle Grazie o Collegiata es una iglesia y catedral de estilo gótico, católica romana de Cortemaggiore, en la provincia de Piacenza, región de Emilia-Romaña, Italia.
La iglesia fue construida en 1480 utilizando diseños de Gilberto Manzi para un plano de cruz latina. El campanario fue añadido por Gaetano Guglielmetti. El interior conserva un valioso políptico, compuesto por doce piezas, de Filippo Mazzola, el padre de Parmigianino. La nave izquierda tiene el mausoleo esculpido y elegante de la familia Pallavicini, incluyendo a Gianludovico y su esposa. Las obras se atribuyen a la escuela de Amedeo. La capilla del Rosario contiene un lienzo de la Madonna, el Niño y San Luis Gonzaga de Pompeo Batoni, y una Pietà restaurada de Il Pordenone. En la cripta hay un fresco que representa a Cristo saliendo de la tumba, atribuido a Antonietto de' Renzi.
