Le district était bordé au début de 1977 par les districts de Land Hadeln, Bremervörde, Osterholz et Wesermarsch ainsi que par la ville libre de Brême, Brême. Au nord-ouest, il était adjacent à l'embouchure de la Weser.
La liste suivante contient toutes les communes qui ont appartenu au district de Wesermünde entre 1932 et 1977, ainsi que toutes les fusions jusqu'en 1977.
