Il comune è situato nella valle omonima minore dell'alta valle Scrivia, a nordest di Genova, adiacente al torrente omonimo che è un affluente di destra del fiume Scrivia. Attraverso il valico di San Fermo, sull'Appennino ligure a 1.129 m s.l.m. presso il monte di San Fermo , è possibile raggiungere la val Borbera e quindi il comune di Cabella Ligure in provincia di Alessandria.
