R1 begynder i nord, som en fortsættelse af motorvejen A1/E19 fra Nederlandene, og kører derefter med uret rundt om sydøst for byens centrum. Den ligger på den tidligere Brialmont-mur, hvor byen Antwerpen i 1957 i sin overordnede plan for den antwerpenske agglomeration havde forudset en bymæssig ekspresvej til lokal tilslutning. I 1958 blev forløbet af R1 sammen med dette af R2 fastlagt ved kongelig beslutning.
