Bad Sassendorf ligger i centrum af Soester Börde ved sydkanten af den vestfalske bugt mellem Haarstrang og det nærliggende Sauerland mod syd samt Münsterland mod nord. I kommunen udspringer Ahse, en biflod til den nordligt beliggende Lippe.
Kommunen Bad Sassendorf består af tolv landsbyer. Disse er Bad Sassendorf, Bettinghausen, Beusingsen, Elfsen, Enkesen im Klei, Heppen, Herringsen, Lohne, Neuengeseke, Opmünden, Ostinghausen og Weslarn.
