Sagter Ems opstår ved sammenløbet af Marka og Ohe i Neuscharrel. Stedet for sammenløbet ligger lige før den sydlige bred af kystkanalen, som Sagter Ems underkrydser.
Derefter krydser Sagter Ems Saterland fra syd til nord og flyder gennem byerne Sedelsberg, Scharrel, Ramsloh, Strücklingen og Bokelesch. Ved landsbyen Roggenberg munder Elisabethfehnkanalen ind i Sagter Ems. Fra dette punkt og indtil sammenkomsten med Dreyschloot ved grænsen til Ostfriesland er Sagter Ems over 0,56 km en federal vandvej, hvor vandvejs- og skibsfartsmyndigheden Ems-Nordsee er ansvarlig. Dreyschloot, som blev bygget kunstigt fra 1848 til 1852, er en forbindelse til Barßeler Tief og Jümme. I Ostfriesland kaldes Sagter Ems Leda.
