Die Stadt liegt nordöstlich von Itzehoe. Durch Kellinghusen fließt die Stör, deren Wasserstand bis etwas oberhalb des Ortes durch die Gezeiten beeinflusst wird. In Kellinghusen beträgt der Tidenhub etwa 1,50 m, in Itzehoe ca. 2,50 m. Ein bis zweimal im Jahr tritt die Stör so weit über die Ufer, dass einige Straßen gesperrt werden müssen. Durch den nach ihr benannten Kellinghusener Ortsteil Mühlenbek fließt die in die Stör mündende Mühlenbek.
