Hohenfelde er en kommune i Kreis Steinburg i Schleswig-Holstein. Kommunen omfatter de delområder Niederreihe, Dauenhof, Glindesmoor, Glindhof, Espe, Halenbrook, Helle, Hörn, Kirchmoor, Oberreihe, Taterbusch, Uhlenflucht og Wisch.
Hohenfelde ligger på en geestausløber mellem Hohenfelder-/Breitenburgermoor og Glindes-/Bokelsessermoor, tæt på motorvej 23, mellem Itzehoe og Elmshorn. Kremper Au samt Horstgraben flyder gennem kommunen. En cykelrute løber mellem Hohenfelde og Horst med Steenbock-broen, som fører over Horstgraben. Gennem kommunen vil motorvej 20 føre, som vil danne et motorvejskryds med motorvej 23. Næste jernbanestationer ligger i Westerhorn i delområdet Dauenhof med jernbanestationen Dauenhof og i Horst, begge stationer ligger i Hamborgs trafikforbund.
