Byen ligger 4 kilometer fra Bad Oldesloe, 5 fra Reinfeld, 15 fra Lübeck og 47 fra Hamburg. Meddewade ligger i et kraftigt kuperet bakket land af en endemorænelandskab, som blev dannet under Weichselisæra. Det består for det meste af permanent græsland. Meddewade har ikke haft skov siden slutningen af 1700-tallet, de sidste træer blev fældet i 1787 for at anlægge marker. Dens højeste punkt ligger i syd ved 51,5 meter, dens laveste i nord ved 2,8 meter over NN.
I nord begrænses det af Trave, i syd af A 1. Meddewade grænser op til Bad Oldesloe, Rethwisch, Barnitz, Feldhorst og Reinfeld.
