Kredsløbet omfatter i det væsentlige den østlige del af halvøen Wagrien, der strækker sig mellem Kieler Bucht og Lübecker Bucht ud i Østersøen i Schleswig-Holsteins bakkeland, samt den foranliggende ø Fehmarn, som er forbundet med fastlandet via Fehmarnsundbroen i forbindelse med Vogelfluglinie. Nabokredse er i vest Kreis Plön og Kreis Segeberg, i syd den administrationsfrie by Lübeck og Kreis Stormarn. Den højeste hævelse er Bungsberg ved Schönwalde. Søbunden af Hemmelsdorfer See er med 39,10 m under NN den dybeste kryptodepression i Tyskland.
Kreis Ostholstein danner sammen med det danske Storstrøms Amt Europaregion Fehmarnbelt, hvor grænseoverskridende projekter kan få særlig støtte.
