Niederelfringhausen ligger syd for kernebyen Hattingen og øst for Langenberg ved landevejen L 924 i grænsen mellem Märkisches Westfalen og Bergisches Land. Syd for byen ligger bydelen Oberelfringhausen; Felderbach flyder gennem begge byer. Den højeste top er Immelberg med 277 m højde. Landskabet kaldes Hattinger Hügelland eller Elfringhauser Schweiz. Der er i Niederelfringhausen beskyttede landskaber som Niederbredenscheid/Elfringhausen og naturreservatet Felderbachtal.
