Byen ligger i øst for Umbrien på den sydlige skråning af Monte Subasio. Efter at den blev grundlagt af umbrerne, kom romerne, der gav den navnet Hispellum. Fra denne periode er der stadig en byport, dele af bymuren og et amfiteater. Spello er en af de bedst bevarede historiske steder i Italien. Ved opførelsen af det primært middelalderlige centrum er der gjort brug af den rødbrune klippe fra Monte Subasio.
