Santi Jacopo e Cristoforo er en lille romersk-katolsk kirke eller kapel beliggende i landsbyen Bolgheri, i kommunen Castagneto Carducci, provinsen Livorno, Toscana, Italien.
En kirke med dette navn befandt sig rundt om året 1000 inden for byens slot. Antipaven Nicolaus V søgte tilflugt her i 1329. Efter at slottet blev ødelagt i 1492, blev den nuværende kirke bygget mellem 1505 og 1515 med sten fra dette slot. Én væg er afledt af middelalderlig bygning. Kirken er helliget San Jacopo Baronto fra Pistoia. Den stenkirke undergik i 1902 en stor restaurering, sponsoreret af grev Alberto Guido. Portalen har et marmor våbenskjold fra familien Della Gherardesca. Interiøret har en faux marmor dekoration, mens adgangen til sakristiet har billeder af de titulerede hellige.
