Optegnelsen af Kassa's eksistens, der stammer fra 1230, er knyttet til eksistensen af rektori-kirken. I processen med bosættelsens transformering fra et landdistriktsfællesskab til en by er alle dens perioder med succeser og fiaskoer blevet afspejlet i St. Elisabeths Katedral.
Ifølge historiske og arkæologiske kilder blev den nuværende katedral bygget på stedet for en tidligere kirke, der også var indviet til St. Elisabeth af Ungarn. Den blev omtalt i dokumenter fra 1283 og 1290, hvor biskoppen af Eger, Andrew II, fritog Košice sogn fra dekanens jurisdiktion.
