Umgeben wird Montsalvy von den Nachbargemeinden Labesserette im Norden, Lapeyrugue im Nordosten, Saint-Hippolyte im Osten, Le Fel im Südosten und Süden, Vieillevie im Südwesten sowie Junhac im Westen.
