Mahlstetten is located on the Großer Heuberg, a high plateau in the southwest of the Swabian Jura, about ten kilometers north of the district town of Tuttlingen.
The surrounding municipalities are Böttingen to the north, Renquishausen and Kolbingen to the east, the town of Mühlheim an der Donau to the south, and Dürbheim to the west.
