El origen del Pequeño Elster se encuentra en el humedal Der Lug, aproximadamente 12 km al este de Finsterwalde, por la confluencia de Mühlengraben y Lugkanal. Históricamente, su origen fue visto por los monjes del monasterio de Dobrilugk en 42 fuentes alrededor del castillo de Sallgast. A partir de aquí, fluye en dirección oeste, pasando cerca de Finsterwalde, aproximadamente a 3 km al norte. En Frankena, su cauce se dirige hacia el suroeste, atravesando Doberlug-Kirchhain y desembocando en Wahrenbrück, al norte de Bad Liebenwerda, en el Schwarze Elster.
En su camino desde el origen hasta la desembocadura, el Pequeño Elster supera una diferencia de altura de solo 30 m, lo que equivale a una pendiente media de lecho de 0,6 ‰.
