Hollern-Twielenfleth est une commune du district de Stade en Basse-Saxe. La commune fait partie de la municipalité de Lühe, qui a son siège administratif dans la commune de Steinkirchen.
La commune est située dans l'Altes Land, la plus grande région de culture de fruits d'Europe, directement sur l'Unterelbe entre Stade et Hambourg. En plus des deux quartiers de Hollern et Twielenfleth, il y a un troisième, Bassenfleth.
