Selon certaines hypothèses, Massafra a été fondée au 5e siècle par des réfugiés de la province romaine d'Afrique, envahie par les Vandales. La première mention historique de la ville remonte cependant au 10e siècle, lorsqu'elle était un gastaldat lombard.
Après la conquête normande du sud de l'Italie, elle a été donnée à un neveu de Robert Guiscard, qui l'a fortifiée et a restauré le château. Plus tard, elle faisait partie du Principauté de Tarente, à laquelle, en tant que ville libre, elle appartenait jusqu'en 1463. En 1484, elle a été assignée à Antonio Piscitello. En 1497, elle a été saccagée par les troupes de Charles VIII de France, et le fief est allé à Artusio Pappacoda, dont la famille l'a détenu pendant un siècle et demi. Ils ont été succédés par les Carmignano et les Imperiale.
