Si origina in diverse fonti a nord del Wolfsberg, alto 580,6 metri, nella regione di Schluckenauer Zipfel, dove si uniscono a 515 metri sul livello del mare presso Panský. A Staré Křečany, un ulteriore affluente proveniente da Zahrady si unisce a lei. Da lì, la Mandau scorre in un arco aperto verso sud fino a Rumburk, situata a est, e continua in direzione sud-est attraverso Seifhennersdorf e Varnsdorf. Successivamente, attraversa un ulteriore tratto dell'Alta Lusazia verso est, dove le sue acque, tra le altre, scorrono attraverso Großschönau, dove confluisce la Lausur, da Hainewalde attraverso la gola di Roschertal fino a Mittelherwigsdorf, dove si unisce il Landwasser e infine raggiunge Zittau, dove sfocia direttamente a est della città a 227,7 m sul livello del mare nella Neiße lusaziana.
