Questa chiesa si trova un po' fuori dal centro storico, sulla strada per la Pota San Jacopo.

Fondata nel 1337, la chiesa ha subito restauri, soprattutto nell'interno a navata unica, in tempi diversi e anche all'inizio del XX secolo.

Sull'altare maggiore una bella tavola di Vincenzo Tamagni, firmata e datata 1522, raffigurante la Madonna col Bambino, San Giovanni Gualberto, San Giovanni Battista, San Benedetto e San Girolamo.

Pregevole il monumento funerario con il busto di suor Fidamonte Malenotti, morta nel 1932.

Nel 1700, quando la chiesa di San Jacopo al Tempio dall'altra parte della strada entrò a far parte dei possedimenti del monastero, fu costruito un viadotto in muratura per consentire alle monache di attraversare la strada senza essere viste da nessuno.
