De Ilse ontspringt aan de helling van de 278 meter hoge Bickplecken en stroomt vanaf haar oorsprong eerst in zuidoostelijke richting, door Welstorf en Kirchheide, bereikt Bredaerbruch en Entrup. Ongeveer vanaf de Steinmühle, ten zuiden van Entrup, stroomt de Ilse daarna in westelijke richting, door Leese en stroomt van daaruit naar het zuiden en mondt bij Lieme bij de Bega-kilometer 12 uit in de Bega. De Ilse stroomt uitsluitend op het grondgebied van de stad Lemgo.
