Rocafort de Queralt is een Spaanse gemeente in de Catalaanse comarca van de Cuenca de Barberá. Volgens gegevens uit 2017 had het een bevolking van 241 inwoners. Het ligt aan de oever van de rivier Vallverd.
Volgens documenten uit 1076 behoorde het toe aan Guerau Guitard de Santa Perpetua, die verantwoordelijk was voor de herbevolking. In de 13e eeuw was het in handen van de familie Queralt en in 1370 werd de baronie van Rocafort de Queralt opgericht. Het wisselde van eigenaar totdat het begin 16e eeuw werd verworven door Joan d'Armengol. De vorige baron was Ferran d'Alemany i de Milà; de baronie kwam vacant te staan na zijn dood in 1954.
De huidige baron is Rafael Cirera i Oller, die de titel herstelde middels een decreet van 2 juni 1995.
