De Santa Maria delle Grazie is een kerk, met bijbehorend klooster, in Milaan uit 1492, die is opgenomen op de Werelderfgoedlijst van de UNESCO.
De kerk dankt deze status aan het fresco van Leonardo da Vinci, voorstellende Het Laatste Avondmaal, dat zich in de refter van het bij de kerk behorende klooster bevindt. Voor het gezicht van Johannes gebruikte Leonardo da Vinci het gezicht van de abt. De kerk werd, samen met een dominicaans klooster, gesticht door Francesco Sforza, hertog van Milaan. De hoofdarchitect van het gebouw was Guiniforte Solari. De apsis van de kerk wordt toegeschreven aan Donato Bramante.
In 1943 werd het complex voor een groot deel in de as gelegd, ten gevolge van geallieerde bombardementen. De muur met daarop Het Laatste Avondmaal bleef staan, ook omdat deze uit voorzorg met zandzakken was verzwaard.
