De Santa Maria della Scala is een titeldiaconie in de wijk Trastevere van de Italiaanse hoofdstad Rome.
De kerk werd gebouwd van 1593 tot 1610 ter ere van een miraculeus icoon van de Madonna. Volgens de traditie bad een moeder voor haar misvormde kind voor het betreffende schilderij dat geplaatst was bovenaan de trap van een nabijgelegen huis. Het kind werd genezen. In de aan Maria gewijde kerk is het icoon te vinden in het noordelijk transept naast een barok beeld van de heilige Johannes van het Kruis. Naast de kerk was in de 17e eeuw een klooster waar de pauselijke apotheek was gevestigd. In 1650 maakte Carlo Rainaldi een Tempietto-vormig baldakijn met 16 Korinthische zuilen en een hoogaltaar.
