Het heeft zijn oorsprong op de noordelijke hellingen van de Serra do Caldeirão en volgt een zuidoost-noordwest koers met een over het algemeen milde helling naar de Atlantische Oceaan, waar het via een kleine rustige delta nabij de stad Vila Nova de Milfontes, 115 kilometer ten zuiden van Lissabon, vernietigt. Het is een van de twee enige rivieren in Portugal met een voornamelijk zuid-noord oriëntatie en een van de weinige rivieren in Europa met deze oriëntatie. Het Mira-bekken grenst aan het Sado-rivierenbekken in het noorden en aan het Guadiana-rivierenbekken in het oosten.
