Die Stadt entstand unterhalb der im 12. Jahrhundert begründeten Burg Altena, auf der im Jahr 1914 durch Richard Schirrmann die weltweit erste ständige Jugendherberge eingerichtet wurde.
Die Stadt liegt im Sauerland im Tal der Lenne. Der höchste Punkt mit 511 m ü. NHN befindet sich am Kohlberg auf der Grenze zur Stadt Neuenrade, der tiefste Punkt am Hünengraben bei 145 m ü. NHN. Das heutige Stadtgebiet hat eine Fläche von rund 44,3 Quadratkilometer; 61 % davon werden von Wald eingenommen.
