Das Dorf befindet sich an einem Berghang auf der Hochfläche der Fränkischen Alb auf einer Höhe von 521 m ü. NHN, sieben Kilometer nordostnördlich von Simmelsdorf.
Durch die Verwaltungsreformen zu Beginn des 19. Jahrhunderts im Königreich Bayern wurde Unternaifermühle mit dem Zweiten Gemeindeedikt ein Teil der Landgemeinde Großengsee, zu der auch das Pfarrdorf Sankt Helena, das Dorf Winterstein, der Weiler Oberachtel sowie die Einöden Mittelnaifermühle, Obernaifermühle und Unternaifermühle gehörten.
