El departamento se creó el 1 de octubre de 1795 tras el voto popular expresado en el País de Lieja el 17 de febrero de 1793. Comprende territorios que anteriormente pertenecían a la principado de Lieja, pero también a la principado de Stavelot-Malmedy y a los Países Bajos austriacos. El departamento del Ourthe corresponde aproximadamente al territorio de una de las provincias de Bélgica, la provincia de Lieja, pero se extiende más al este que esta, en la actual Alemania, hasta Schleiden y Kronenburg, con enclaves en el departamento de Sarre. Su capital es Lieja.
