Sarnano sorge ai piedi dei Monti Sibillini, sovrastato a ovest dai picchi più settentrionali delle catena, i quali dal lato orientale mostrano le loro irte pareti rocciose, solcate da aspre vallate e gole. Il borgo è situato su un colle alla destra del torrente Tennacola, su una lingua di terra racchiusa tra quest'ultimo e il suo affluente, il Rio Terro. Il territorio, prevalentemente collinare, spazia a oriente tra valli, boschi e campi coltivati. Verso nord i rilievi digradano nell'antico piano lacustre di Pian di Pieca risalente al Quaternario.
