De ongeveer negen kilometer lange rivier maakt deel uit van het riviersysteem van de Elbe en is van geringe diepte. Hij ontspringt in Westerende, stroomt oostwaarts door het dorp en mondt - nadat hij talrijke zijrivieren heeft opgevangen - bij Kathusen uit in de Medem. Deze mondt op haar beurt na nog eens 17 kilometer samen met de Elbe-Weser-scheepvaartroute bij Otterndorf uit in de Neder-Elbe.
