Den store by Hohenstein-Ernstthal ligger i den østlige del af Zwickau-distriktet. Byen er især kendt som fødestedet for forfatteren Karl May og gennem Sachsenring, hvor blandt andet Tysklandsmesterskabet i motocross afholdes.
Den tidligere kommunecenter Glauchau ligger ca. 15 km mod vest og byen Chemnitz ca. 20 km mod øst for byen. Hohenstein-Ernstthal ligger på grænsen mellem den nordlige kant af Erzgebirgsbækken og den sydlige kant af Mellemsaksisk løssjord-bakker, som her når op til Pfaffenberg-højden. Da en stor del af byens centrum ligger på den sydlige side af Pfaffenberg, kan man herfra se store dele af Erzgebirge. NORD for byen findes Oberwald-dæmningen og Oberwald serpentinbrud. De tilhører landskabsbeskyttelsesområdet "Pfaffenberg-Oberwald". I bydelen Ernstthal udspringer Goldbach, som afvander til Zwickauer Mulde via Lungwitzbach. Den højeste punkt i Wüstenbrand er Heidelberg med sit udsigtstårn.
