Den har sine kilder på de nordlige skråninger af Serra do Caldeirão og følger en sydøst–nordvestlig kurs med en generelt mild hældning mod Atlanterhavet, hvor den udløber gennem en lille rolig delta nær byen Vila Nova de Milfontes, 115 kilometer syd for Lissabon. Det er en af de to eneste floder i Portugal med hovedsageligt syd–nordlig orientering og en af de få floder i Europa med denne orientering. Mira-bassinet grænser op til Sado-flodbassinet mod nord og Guadiana-flodbassinet mod øst.
