Dit leidde tot zijn roman De Toverberg (1924), waarin hij het leven in een sanatorium beschrijft als een microkosmos van filosofische en existentiële reflecties. Het boek speelt zich af in het fictieve "Berghof"-sanatorium, gebaseerd op Davos, en behandelt thema's zoals ziekte, dood en de tijd.
